JOB DESCRIPTION
- Provide medication counseling and drug information in compliance with clinical pharmacy regulations and professional standards.
- Prepare clinical pharmacy activity reports; review medication recommendations with prescribers and present relevant findings at interdisciplinary meetings.
- Analyze and evaluate medication utilization and participate in developing the hospital formulary.
- Develop and monitor appropriate use protocols for formulary medications, including narrow therapeutic index drugs, antibiotics, high-alert medications, and medications associated with significant adverse effects.
- Provide guidance and supervision on medication use; organize drug information activities and clinical pharmacy training programs.
- Monitor and report medication utilization and adverse drug reactions (ADRs).
- Participate in multidisciplinary consultations for critically ill patients, special medications, and infections caused by multidrug-resistant organisms.
- Participate in scientific research, quality improvement initiatives, and activities aimed at optimizing medication therapy outcomes.
- Stay updated on pharmaceutical and clinical pharmacy regulations and advise management on hospital-wide implementation plans.
- Monitor compliance with high-alert medication requirements and medication management and use standards in accordance with JCI standards and the criteria of the Ministry of Health.
- Perform other duties assigned by the Pharmacy Department Manager or Medical Director.
Customer Service (Internal & External)
- Collaborate with multidisciplinary teams to manage medication therapy and optimize treatment outcomes for patients.
JOB REQUIREMENTS
- Bachelor's degree in Pharmacy or higher and a valid Pharmacy Practice License.
- At least one year of clinical pharmacy experience in a hospital setting.
- Strong knowledge of hospital medication systems, clinical pharmacy practices, regulations of the Ministry of Health and Department of Health, and JCI standards.
- Ability to guide clinical teams in accordance with internationally recognized professional guidelines.
- Strong communication and multidisciplinary collaboration skills.
- High sense of responsibility, with good team management and work organization skills.
- Ability to communicate in English and read professional and scientific documents in English.
- Proficiency in Microsoft Office applications.
